The NHS Health Check programme aims to help prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions or has certain risk factors, can request to be invited (ideally once every five years) to have a check to assess their risk of developing these conditions.
If identified to be at a higher risk than average you will be given support and advice to help reduce or manage that risk.

If you have been invited for a health check we would appreciate it if you could complete our health questionnaire and bring it with you to your appointment. A copy can be downloaded from the menu below along with a copy of the invitation letter.
